Survodutide: weight and liver trials and status in France

Survodutide: weight and liver trials and status in France
NutritionImage de l’article

A page offering survodutide for sale does not prove that it is an available medicine. As of October 10, 2026, survodutide remains an experimental treatment: no marketing authorization was identified in the French and European registers consulted. Clinical trials provide a regulated access route for selected patients.

The human results nevertheless deserve careful consideration. Phase 3 publications show weight loss and a reduction in liver fat. But losing weight, reducing liver fat, improving inflammation and reversing fibrosis are different outcomes. No striking percentage makes them interchangeable.

Survodutide and BI 456906: two receptors, a hypothesis to test

Survodutide and BI 456906 refer to the same molecule. It is a GLP-1 and glucagon receptor agonist. The GLP-1 component contributes to appetite and blood glucose regulation. The glucagon component offers a potential way to modify energy and liver metabolism. The mechanism and its limitations are described in the SYNCHRONIZE-1 publications and those of SYNCHRONIZE-MASLD.

This dual targeting does not mean that the liver is “repaired” independently of weight loss. A trial measuring both effects together is not enough to isolate each receptor’s contribution. A plausible biological explanation must be distinguished from a demonstrated clinical benefit.

Tirzepatide targets GLP-1 and GIP, a different combination. The number of receptors is not a measure of efficacy. To understand another GLP-1/glucagon candidate, the article on mazdutide and its status in France · FR provides additional context, without ranking the molecules.

Trial table: in whom, against what and for how long?

The doses below identify the research groups. They are neither a prescription nor a dosing regimen for use. The trials are randomized, placebo-controlled and double-blind.

Trial and populationComparator and durationKey resultTolerability and limitations
Obesity, phase 2, 2024 : 386 treated adults, without diabetes, BMI ≥27Placebo, 46 weeksAt 4.8 mg, weight change of −14.9% versus −2.8%, according to planned treatmentGastrointestinal events: 232/309 receiving survodutide versus 32/77 receiving placebo. Only 233/386 completed the treatment period. Abstract consulted; full text unavailable
SYNCHRONIZE-1, phase 3, 2026 : 725 treated adults, without diabetes, with obesity or overweight with a complicationPlacebo and lifestyle support, 76 weeksAnalysis accounting for discontinuations: −12.2% at 3.6 mg and −13.0% at 6.0 mg, versus −5.4%Drug discontinuations due to adverse events: 57/241, 60/242 and 13/242. No active comparator
MASH, phase 2, 2024 : 293 treated adults, biopsy-confirmed MASH, F1 to F3 fibrosisPlacebo, 48 weeksMASH improvement without worsening of fibrosis: 47%, 62%, 43% at planned doses of 2.4, 4.8 and 6.0 mg, versus 14%Discontinuations due to adverse events: 44/219 receiving survodutide versus 2/74. Histological endpoint, with no evidence of prevention of long-term complications
SYNCHRONIZE-MASLD, phase 3, 2026 : 216 treated adults, with obesity and at-risk MASLD, with or without diabetesPlacebo, 48 weeksAnalysis accounting for discontinuations: 68.5% versus 28.6% achieved a relative reduction of at least 30% in liver fat on MRIFrequent gastrointestinal events. All-cause discontinuations: 60/146 versus 28/70. The primary endpoint is not fibrosis regression on biopsy

These rows address different questions. A trial’s phase is no substitute for examining its primary endpoint. A phase 3 trial focused on MRI does not automatically confirm a phase 2 histological result. The results in the first row remain a historical reference drawn from the abstract, without detailed interpretation of the unavailable analyses.

Obesity: why 13% and 16.6% can come from the same trial

SYNCHRONIZE-1 includes adults with a BMI of at least 30, or at least 27 with an obesity-related complication. Diabetes is excluded. Participants also receive dietary and physical activity support: placebo therefore represents a supported care pathway, not the absence of intervention.

Two methods address two questions. The analysis called treatment-regimen accounts for discontinuations and the use of other obesity treatments, even those prohibited by the protocol. The analysis called efficacy estimates a hypothetical situation in which participants follow their assigned treatment without using these other medicines. It uses modeling, not simply the average among those who remain.

At 6.0 mg, this second analysis gives −16.6% versus −3.2%, while the primary analysis gives −13.0% versus −5.4%. These figures do not contradict each other: they do not address exactly the same question. The headline “up to 16.6%” becomes misleading if it removes this distinction. Full text of SYNCHRONIZE-1.

The difference from placebo is more relevant than the raw reduction for estimating the effect specific to the strategy studied. It remains a group average, not an individual prediction. Weight alone tells us nothing about tolerability, quality of life, long-term maintenance or complications prevented.

The program also includes SYNCHRONIZE-2, in adults with type 2 diabetes. Its results must not be pooled with those of SYNCHRONIZE-1. The registry was consulted, but the full text of its recent publication could not be verified here: no efficacy figures from this trial are reported.

MASLD and MASH: fat, inflammation and fibrosis are not synonyms

MASLD combines hepatic steatosis with a metabolic context. MASH also involves inflammation and liver cell injury. Fibrosis is an accumulation of scar tissue, classified by stage. Cirrhosis is an advanced stage, not a synonym for “fatty liver.” These distinctions are detailed in the SYNCHRONIZE-MASLD publication.

An example helps avoid a common misunderstanding: if fat content falls from 20% to 14%, its relative reduction is 30%. This does not mean that 30% of the liver has healed or that its fibrosis has decreased. It is a fat measurement with a specific denominator.

What the phase 2 biopsy measures

The 2024 MASH trial defines improvement as a decrease in the activity score, with improvement in inflammation or cell ballooning, without an increase in fibrosis stage. MASH improvement and MASH resolution are two distinct endpoints. Likewise, “without worsening of fibrosis” does not mean “fibrosis regression.”

Rates of regression by at least one fibrosis stage are 34%, 36% and 34%, versus 22% with placebo. This is a secondary signal requiring confirmation, not evidence of prevention of cirrhosis or transplantation. The often-highlighted figure of “83%” comes from a different analysis, based on the dose actually received and paired biopsies. It does not replace the rates in the table, which are based on assigned doses and treat missing data as nonresponses. MASH trial, full text.

What the phase 3 MRI measures

SYNCHRONIZE-MASLD mainly measures liver fat and weight. Its participants are not all selected on the basis of a biopsy showing MASH with significant fibrosis. An MRI improvement in this population therefore does not demonstrate histological resolution of advanced MASH.

The study reports changes in markers of inflammation and stiffness, with results that are not consistent across all methods. This reinforces the value of follow-up while preventing all these indicators from being summarized as “the liver improves.” The article on GLP-1 and fatty liver trials · FR explains why each molecule, population and endpoint must be examined separately.

Adverse events and discontinuations: use the right denominators

Nausea, vomiting, diarrhea and constipation are common. The discontinuations in the table concern treated participants, with different sample sizes across trials. All-cause discontinuation rates cannot be presented as rates of discontinuation due to toxicity.

In SYNCHRONIZE-1, stopping the drug and leaving the study are two distinct events. A participant may stop injections while remaining under follow-up. Discontinuations due to gastrointestinal events are themselves a subset of discontinuations due to adverse events. The safety table in the full text separates these categories. This distinction helps explain the gap between the efficacy analyses.

The trials do not cover every patient profile. SYNCHRONIZE-1 notably excludes certain histories of pancreatitis, medullary thyroid carcinoma, multiple endocrine neoplasia type 2, uncontrolled hypertension and recent cardiovascular events. These exclusions are not an official summary of contraindications for an authorized medicine. They limit the generalizability of the results. Pregnancy, breastfeeding, comorbidities and concomitant treatments require medical assessment, never personal extrapolation from the protocol.

France: authorization, clinical trials and offers to purchase

Access status checked on October 10, 2026. No medicinal product containing survodutide was found in the medicinal product and composition files of the French public medicines database, dated September 29, 2026, or in the European register of active authorizations consulted. No authorized brand name in France was identified. This check does not guarantee that the status will remain unchanged after that date.

You can find the sources used for this check in the downloadable French files and the European Commission register. The EMA pediatric decision concerning survodutide concerns its development: it is not a marketing authorization.

For research, the LIVERAGE and LIVERAGE-Cirrhosis records, updated on October 1, 2026, list sites in France. In LIVERAGE, the French sites consulted are listed as not yet recruiting. In LIVERAGE-Cirrhosis, Beaujon in Clichy, Dijon and Croix-Rousse in Lyon are listed as recruiting. An overall “recruiting” status does not mean that every site is recruiting.

LIVERAGE targets MASH with F2 or F3 fibrosis. LIVERAGE-Cirrhosis concerns compensated cirrhosis, with additional inclusion criteria. Steatosis found on ultrasound is therefore not enough to consider yourself eligible. Local status may change: the specialist and study team must confirm it. Participation involves consent, follow-up and the possibility of receiving placebo.

A commercial offer does not replace any of these pathways. The ANSM alert of July 2, 2026, reproduced by the French National Order of Pharmacists, highlights the uncertain composition of peptides sold outside official channels. A “research use only” label is not a guarantee for human use. If you have already used them and experience adverse effects, seek medical care promptly. In a life-threatening emergency, call 15 or 112 in France.

Questions to prepare for your doctor

The consultation should start with your diagnosis and goals. Here are useful questions to bring:

  • Is my main problem obesity, diabetes, MASLD or MASH with fibrosis?
  • Which test clarifies my liver risk, and what are its limitations?
  • Which already-authorized options match my indication and medical history?
  • If a trial is relevant, which site is actually recruiting, and what criteria must I meet?
  • Which outcome will be monitored: weight, fat on MRI, histology or clinical complications?
  • Which events should prompt me to contact the team, and how does follow-up continue after discontinuation?

No self-administration protocol can be drawn from this article. The useful decision is to obtain an appropriate assessment and, potentially, information about a genuinely accessible trial.

Le Biohacker’s verdict

Survodutide has encouraging clinical results, with gastrointestinal tolerability affecting treatment continuation. Confidence is high regarding superiority over placebo for weight in the SYNCHRONIZE-1 population and for fat on MRI in SYNCHRONIZE-MASLD. It remains more limited for fibrosis and insufficient, in these publications, to conclude that long-term liver complications are prevented.

The pivotal studies are funded by Boehringer Ingelheim, with sponsor involvement in their conduct and publications. Their results must be read alongside their methods and limitations. My editorial choice is to prioritize analyses accounting for discontinuations and reject any indirect ranking against tirzepatide. In France, the immediate decision remains a consultation and an eligibility check, without purchasing an experimental product.
