Four cohorts, not a randomized trial
The researchers pooled observational studies involving adults with confirmed gout. Eligible studies had to distinguish exposure to allopurinol and report adjusted hazard ratios for the risk of myocardial infarction.
The literature search covered PubMed, Scopus and Web of Science through January 2026. Four retrospective cohorts were included. The supplied abstract does not specify the total sample size, follow-up duration or detailed characteristics of the groups compared.
A favorable main finding that depends on analytical choices
In the main analysis, allopurinol use was associated with a hazard ratio of 0.84, with a 95% confidence interval of 0.76 to 0.94. This relative result does not allow us to calculate how many heart attacks might be prevented: the abstract does not provide the necessary absolute event rates.
When the researchers removed the study carrying the greatest weight in the calculation, the estimate shifted to 1.04, with a confidence interval of 0.79 to 1.36. The favorable association therefore disappeared. Restricting the analysis to studies using an active comparator yielded the same result.
Another method for calculating uncertainty, the Hartung-Knapp adjustment, kept the estimate at 0.84 but widened the confidence interval to 0.71–1.00. The authors report that this result was no longer statistically significant.
What this review tells us
The abstract’s conclusion highlights a modest and significant reduction in heart attack risk. The sensitivity analyses call for a more cautious reading: the main signal does not hold up under several tests of its robustness.
These findings do not prove the absence of an effect either. Uncertainty remains, particularly because differences between patients can persist in observational cohorts despite statistical adjustments. This review is not sufficient to justify using allopurinol to prevent heart attacks.
Limitations to keep in mind
Only the PubMed abstract was consulted, not the full text. Sample sizes, follow-up durations, absolute risks and details of adjustments are not available in this dossier. Funding and conflicts of interest have not been verified.
What we can take away
A cardiovascular benefit of allopurinol in adults with gout remains uncertain. The favorable association in the main analysis is too fragile to conclude that it protects against heart attacks.
